When an elderly person checks out of a nursing home or a hospital, he/she gets readmitted within a very short time. This is mostly because during the transition period, some things were not taken care of as they should be. The pending issues could be medications, doctor's appointments, or chronic illnesses. Statistics show that more than one senior person out of five gets readmitted within one month of discharge.
